Health Issues

This guide will cover how to view and record Health Issues within a service users Care Plan on Care Control Cloud.

From the Care Plan Summary screen, open the Medical menu.

Select Health Issues.

Health issues can be filtered by Current, Historic and All issues.

The left-hand side of the screen displays a body map to locate the area affected by the health issue.

The right-hand side of the screen shows the overview details of the health issue as well as attached images.

You can also open the Treatment Plan and Notes using the respective tabs.

Press New Health Issue to record a new one.

A new form will open. Complete each section, including the Summary, Description and Treatment Plan.

You can also attach photos if necessary. Finally, mark where the health issue is on the body map.

After completing the form, press Save Changes.

Your new health issue will be added to the care plan.

You can update and close the issue by pressing Review Health Issue.

The form will open again. Here, you can amend any of the details originally recorded.

You can also add an Associated Note as well as Close the Health Issue via the buttons in the bottom left corner.

After making any changes here, make sure to press Save Changes.

If you chose to add an Associated Note, a pop-up note window will open. Here, add any updates to the issue and press Save Note to Health Issue.

If you chose to Close the Health Issue, you will be required to enter a reason for closing it.

Then, press Close Health Issue. This will move the record into the Historic Health Issues.
