Proposing a Care Review Change – Care Control Learning Hub

Proposing a Care Review Change

Care Control Pocket contains a proposal feature which allows staff members to suggest edits to Care Review sections. These can then be reviewed on Care Control Windows and implemented into a section if the change is accepted. This guide will cover how to propose a change as well as action it on CC Windows.

1

On Care Control Pocket, open the service user list by pressing the arrows icon in the bottom left corner.

2

Select a service user and press continue.

3

Select Care Plan at the bottom of the service user summary screen.

4

Choose Care Review from the options.

5

You can navigate through the different sections of the review via the drop-down list at the top of the screen.

To Propose a Change, press the button at the bottom of the care review section.

6

You will enter edit mode. Simply press the blue edit icon to suggest a text change.

7

Amend, add or remove the text in this care review section.

Press OK after making any changes.

8

After making edits, add a signature by pressing the button at the bottom of the screen.

9

Simply draw on the screen to sign. Press Save Record after doing so.

10

Finally, press Save when you are happy with your changes.

11

Confirm the proposal by pressing Yes.

12

In the Administration of Care Control Windows, navigate to the Care Review Changes tool, which can be found under the Care Planning tab.

13

Here, you can view all proposals to your service users Care Reviews.

Begin by selecting a service user who has a proposed change.

14

Next, select the section which has the proposed change.

15

The Current and Proposed detail will show on the left and right respectively. The current shows what is currently on the care review, and the proposed shows the changes recorded on CC Pocket.

We can Accept or Reject the proposal via the Action buttons in the top right of the window.

16

In this example, we Accepted the change. A confirmation message will be displayed. Press OK.

17

If you accepted the changes, you can process an Interim Care Review. This creates a new version of the care review with the changes applied to it – but, the due date of the next care review will not change.

Tick the Interim Review box if you want this to occur, then press Process Changes.

18

You will receive a confirmation message. Press Yes.
