Amending Life History
Sections and Questions

This guide will cover how to amend the content of the Life History sections and questions via Care Control Windows. These can be customised to best fit your organisations and the information you want to record.

In the Administration of Care Control Windows, navigate to the Life History Sections tool, which can be found under the Care Planning tab.

A new window will open. Here, a list of your existing sections will be displayed. You can amend the Title and Order of existing categories by typing in the text boxes.

You can also Delete a section by selecting it and pressing Delete Section.

To add a new section, simply type the Title and Order Number in the blank row at the bottom of the list.

Press Save Changes after making any amendments.

We can now amend the questions in each section. Navigate to the Life History Questions tool, also found under the Care Planning tab.

A new window will open. Here, a list of existing questions will be displayed. Each is contained within a section and ordered via the order number.

You can amend the section and question content of existing ones, simply by typing in the text boxes.

You can also archive a question by ticking the check box on the right hand side.

To add a new one, scroll to the bottom of the list.

Here, in the blank row, select a Section and enter the Question Text in each of the text boxes. Finally, add an Order Number.

After making any amendments, press Save Changes.
