Open the Admin App
Click Visit under Care Planning
Open the Risk & Care Review dropdown
Pick Generic Risk Assessments
Click Add New Risk Assessment
Choose Existing Risk Assessment as Template
Enter the Name of the Risk Assessment
Then click Browse Files to Upload
Double Click the file you want to upload
Once the file is selected, click Save
