How to add Impairments

This guide explains how to record and manage Impairments and Adjustments in the Care Planning App. Accurately completing this section ensures that important personal and accessibility details are correctly stored and shared when care information is exchanged with other systems, such as those used by the NHS.

The Minimum Operational Data (MOD) Standard, developed by NHS England’s Digitising Social Care Programme, sets out the essential information that adult social care providers must record to ensure consistency, quality, and safe data sharing across the health and care sector. By following this guide, you’ll help your service meet DSCR (Digitising Social Care Records) compliance standards and maintain alignment with NHS interoperability requirements.

Recording impairments and reasonable adjustments not only supports legal and clinical obligations under the Equality Act 2010, but also helps ensure that every person receives care tailored to their individual needs. This process contributes directly to safer, more personalised, and fully auditable digital records across health and social care.

Enter the Care Planning App

Click Visit under Resident Care Plans

Open the Essentials dropdown

Open Personal Details

Click the Impairments tab at the top

Click Edit Care Plan

Open the Impairment Type dropdown

Select the Impairment Type from the list

Enter additional information in the Further Details box

Open the Adjustment Category dropdown

Select an Adjustment Category option

Open the Adjustment Option dropdown

Select the Adjustment Option from the list

Enter text into the Adjustment Details tab

Select Add Adjustment to confirm

Add additional Impairments if required

Click Save Changes when complete

To remove an Impairment, click Edit Care Plan

Click Delete Impairment to remove it

You can also Undo Deletion if needed

Click Save Changes to confirm
